At Western Avenue Family Dental, Oral Surgery & Implant Center, we strive to provide the best care and service to all our patients. To ensure that we can accommodate the needs of all our patients and maintain an efficient schedule, we have established the following cancellation policy:
1. Notice Requirement: We request that you provide at least 48 hours’ notice if you need to cancel or reschedule your appointment. This allows us to offer your appointment slot to another patient who may be waiting for care.
2. Late Cancellations: Cancellations made less than 24 hours before the scheduled appointment may be subject to a cancellation fee. This fee will be determined based on the length of the appointment and the circumstances of the cancellation.
3. No-Show Policy: If you miss your appointment without notifying us, you will be considered a no-show; A no-show fee will be applied to your account. Repeated no-shows may result in changes to your scheduling privileges.
4. Emergencies: We understand that emergencies and unforeseen circumstances can occur. If you need to cancel or reschedule due to an emergency, please contact us as soon as possible. We will review these situations on a case-by-case basis and may waive the cancellation fee.
5. Appointment Confirmation: To help you remember your appointment, we provide reminders via phone, text, or email. However, it remains your responsibility to keep track of your appointments and notify us if changes are needed.
6. Fee Details:
Cancellation and no-show fees are as follows:
Less than 24 hours’ notice: $40.00
No-show: $50.00
No-show Surgery Appointments: $100.00
7. Payment of Fees: Cancellation and no-show fees will be applied to your account and must be settled prior to your next appointment.
